Specialist Imaging Partners Pty Ltd (we) are committed to the protection of your personal and health related information, in accordance with the Privacy Act 1988 (Cth) (Act) and the Australian Privacy Principles (APPs).

This privacy policy explains how we collect, use, hold and disclose your personal information, including your health information and other sensitive information.

What information do we collect?

Personal information is defined in the Act as information (including health information) or opinions that identifies a person or whose identity can be reasonably ascertained from the information. Below is a list of some (but not all) of the information we may collect from you.

Examples of personal information we may collect:

  • Name, address, contact details (telephone and email);
  • Date of birth;
  • Gender; and
  • Emergency contact details for your nominated emergency contact person.

Examples of sensitive information and health information we may collect:

  • Medical history, test results (e.g. blood test results and pathology results) and results of previous scans;
  • Information that has been provided by a referring medical practitioner or specialist;
  • Medical provider/s details, such as your general practitioners and specialist practitioners or allied health professionals;
  • Financial information, such as credit information, banking details and credit card numbers;
  • Details of private health insurance arrangements;
  • Medicare number, pension number, Veteran Affairs details and concession cards;
  • WorkCover claim numbers and claim details if necessary;
  • Information that has been provided on your behalf when authorised (for example, a family member or authorised representative); and
  • Any other information that we may collect directly from you (either verbally or in writing) or information that we have collected from you when you have called us, written to us, or visited us.

Purpose for collecting personal information?

We collect your personal information for the primary purpose of providing medical sonography services to you as our patient. However, we may also use your personal information for secondary purposes closely related to the primary purposes, in circumstances where you would reasonably expect such use or disclosure. This includes:

  • providing advice to a referring medical practitioner or specialist in relation to the results of the services provided to you;
  • sending you correspondence in relation to past or upcoming services (for example, reminding you of an appointment or asking you to update your details);
  • customising services and provide educational resources for clinicians, practice managers and auxiliary staff;
  • ensuring patient medical records are correctly linked with relevant healthcare providers;
  • undertaking quality assurance exercises (such as audits, accreditations, training or complaint handling); and
  • processing internal administrative procedures such as invoicing and billing.

Direct marketing

We may also use your personal information for marketing. However, you may unsubscribe from our mailing/marketing lists at any time by contacting us in writing.

How do we collect your personal information?

Collection of personal information may occur throughout the course of us providing you with the services, such as:

  • at the time of contacting us to make an enquiry or to book an appointment over the phone or online;
  • when completing/signing the patient information/consent forms;
  • on first contact with a staff member;
  • at the time of billing you for the services;
  • from your health service provider, practitioner, specialist, allied health professional, private health insurer or Medicare;
  • from your family (or someone who is authorised to act as your representative); and
  • any other sources as necessary to provide the services.

Please note that if we receive unsolicited personal information about you which we would not normally have collected and this information is not relevant to us providing the services to you, we will destroy or de-identify the information, subject to your consent.

How do we hold your personal information?

We will hold your personal information securely in accordance with the requirements under the Act. We will take all reasonable measures to ensure that your personal information is protected from misuse, interference, loss and from unauthorised access, modification and disclosure. This includes:

  • having appropriate policies, procedures and training in place for staff;
  • implementing security procedures for business premises and IT systems; and
  • ensuring sensitive personal information can only be accessed by authorised users for the purpose of providing the relevant health service.

What happens if you do not provide your personal information to us?

Unfortunately, if you do not provide your personal information (including sensitive information) we may be unable to provide you with the services you request due to safety and legal requirements. Further, due to the nature of the services, it is not possible for you to deal with us anonymously or by using a pseudonym.

Third party providers

We may from time to time have arrangements with third party providers located both here and outside of Australia which may deal with your personal information such as for the purposes of providing medical reporting, storage, data and communication services to us. In engaging third party providers, we will take all reasonable steps to protect your personal information from misuse, interference, loss and from unauthorised access, modification or disclosure and to ensure that these third party service providers are compliant with the APPs.

Updating your information

It is an important to us that your personal information is up to date. We will take reasonable steps to make sure that your personal information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide quality services to you.

Access to personal information

You can request access to personal information we may hold about you. There is no charge for lodging a request to access your information. However, we may make a small charge for our time involved in providing this information and for associated costs such as photocopying.
